A Singer’s Masterclass with Anju Panta — SM Studio

By submitting this registration form, I acknowledge and agree to the following:

1. Registration & Participation

Submitting this form does not automatically guarantee participation in the workshop. Registration is subject to availability and final confirmation by SM Studio.

SM Studio reserves the right to accept, decline, or cancel a registration when reasonably necessary.

2. Participant Information

I confirm that the information provided in this registration form is accurate and complete.

I understand that SM Studio may contact me by phone, text message, or email regarding my registration, workshop information, scheduling, preparation instructions, or other information related to this event.

3. Workshop Conduct

I agree to behave respectfully and professionally toward Anju Panta, SM Studio staff, other participants, guests, and the studio facility.

Disruptive, inappropriate, unsafe, threatening, or disrespectful behavior may result in removal from the workshop without further participation.

Participants are responsible for following instructions provided by the instructor and SM Studio staff during the workshop.

4. Personal Belongings

Participants are responsible for their own personal belongings, instruments, devices, and other property brought to the workshop.

SM Studio is not responsible for lost, stolen, misplaced, or damaged personal property except where liability cannot legally be excluded.

5. Health, Safety & Participation

I understand that participation may include singing, vocal exercises, standing, performing, studio recording activities, and other normal workshop activities.

I agree to participate only to the extent that I am comfortable and physically able to do so and to notify SM Studio staff if I require assistance or accommodation.

6. Photography, Video & Recording

SM Studio may photograph, film, or otherwise document portions of the workshop.

Where I have provided media consent through the registration form, I authorize SM Studio to use photographs, video, or audio recordings containing my image, voice, or participation for educational, promotional, social media, website, archival, or marketing purposes.

If I choose not to provide media consent, SM Studio will make reasonable efforts to respect that choice.

Participants may not commercially record, reproduce, livestream, distribute, or publish workshop instruction or materials without prior permission from SM Studio.

7. Workshop Materials & Intellectual Property

Any educational materials, handouts, exercises, recordings, presentations, or other materials provided during the workshop are intended for the participant’s personal educational use unless otherwise stated.

Participants may not reproduce, sell, publicly distribute, or commercially use workshop materials without authorization from SM Studio or the applicable rights holder.

8. Schedule or Event Changes

SM Studio may make reasonable changes to the workshop schedule, activities, instructor format, or other event details when necessary because of circumstances outside its control.

If the workshop is postponed, rescheduled, or canceled, registered participants will be contacted with available options and updated information.

9. Cancellation & Registration Policies

Any cancellation, refund, transfer, or registration policies communicated by SM Studio as part of the participant’s confirmed registration will apply to that registration.

Participants are responsible for reviewing those policies before completing any required payment or final confirmation.

10. Minors

Participants under 18 years of age must have registration completed or approved by a parent or legal guardian.

By submitting registration on behalf of a minor, the parent or legal guardian confirms that they have authority to provide the required consent and accept these terms on behalf of the participant.

11. Assumption of Ordinary Workshop Risks

I understand that participation in an in-person music and vocal workshop involves ordinary risks associated with group activities and use of a professional studio environment.

I voluntarily choose to participate and agree to follow reasonable safety instructions provided by SM Studio.

Nothing in these terms is intended to waive rights or responsibilities that cannot legally be waived under applicable law.

12. Privacy

Information collected through this registration form will be used primarily for registration administration, participant communication, event planning, and related SM Studio purposes.

SM Studio will not intentionally sell participant registration information to third parties.

Information may be shared with service providers when reasonably necessary to operate registration, communications, payment processing, or the event.

13. Electronic Agreement

By checking the agreement box and submitting this online registration form, I confirm that I have read, understood, and agree to these Terms & Agreement.

My electronic acceptance will be treated as my agreement to these terms.
